It has something to do with the Helm of Winterhold and/or Driftshade Refuge, neither of which I'm able to get any quests for, and basically it's annoying as fuck because I'm stuck as a werewolf (which really isn't all that bad)... hopefully the upcoming patch fixes this. You use the witch head at the last companion quest. Hold onto it.
